Complete the On the Job Injury Report for job related injuries. If non-emergency medical attention is required you will need to visit a Concentra Urgent Care facility after notifying your supervisor. Your supervisor must complete a Medical Treatment Referral Form for each visit to the doctor. A “Vehicle Accident Report” must be completed and submitted to Risk Management in addition to the “On the Job Injury Report” if your injury is the result of a vehicle accident.
